Reporting Safety or Quality Issues
You have the right to present concerns pertaining to patient care and safety at all VISN 20 Health Care Network locations including Outpatient Clinics. These concerns should be directed to the Manager of the Service or Department that is providing care, the patient advocate or the facility director. If you are unsure of whom to contact, any employee can direct you to the appropriate supervisor.
Should it appear that your concerns can’t be resolved at the local facility, you should contact VISN 20 at:
VISN 20 Network Office
1601 E. 4th Plain Blvd, Bldg 17, Suite C403
Vancouver, WA 98661
Phone: (360) 619-5925
If at any time you feel that hospital management has not addressed or resolved the concern, you may contact the Joint Commission.
