New Proposal or Re-Submission ( click here for NIH Continuation or Renewals)

		1. Research Proposal to R&D Office with required materials: MOU VA PPQ OHSU PPQ IRB Materials IACUC Materials Bio-Safety Materials Administrative Review Materials

		2. Clinical Service Chief signs PPQ to approve Administrative Review (if applicable)

		3. ACOS/R&D Signs PPQ Proposal Returned to PI

4. Institutional Review Board (IRB)	4. Institutional Animal Care & Use Committee (IACUC)	4. Subcommittee on Research Safety (SRS)	4. HSR&D Scientific Review Committee (Health Services Studies Only)	4. OHSU Cancer Institute Review (Cancer Studies Only)

		5. R&D Committee (R&D)

		6. PI Notified Project can Begin

NIH Continuation or Renewals

	1. Proposal and OHSU PPQ to R&D Office (VA PPQ not required)

	2. OHSU PPQ signed by ACOS/R&D

	3. Proposal & PPQ returned to PI
