Community Reintegration Services - VASH
The Goal
The goal of the VA Supported Housing (VASH) program is
to show that appropriate health and supportive services
combined with case managed, safe and affordable housing,
can help homeless veterans with severe psychiatric and/or
substance abuse disorders lead healthy, productive lives
in the community: abandoning life on the streets.
The Method
The Program has been awarded a grant to provide Section
8 vouchers and case management services for 25 homeless
veterans in the Portland Metropolitan area.
Case Management Services Include
- Linking participants to inpatient hospitalization;
- Crisis intervention;
- Substance abuse
- Group and individual treatment
- Assistance in locating appropriate housing;
- Facilitation of referrals to community agencies for
vocational services, educational pursuits, and other
assistance as appropriate.
Eligibility Requirements:
- Veteran status - those eligible for services under
Veterans
Health Administration guidelines;
- Homeless;
- Diagnosed with a severe substance abuse and/or psychiatric
disorder;
- Demonstrated period(s) of stabilization;
- Commitment
- Willingness to participate in the case management
component of the program;
- 90 days clean and sober.
