VISN 20 Data Warehouse Research Study Tracking

The Data Management Team maintains a study tracking file for all active projects, for the file the following documents are required:

Data Warehouse Navigation
Overview and History
MetaData
ER Diagrams
VISN 20 Cubes
Home

 

1)  A copy of the study protocol.
2)  A copy of the IRB approval.
3)  A copy of local R&D approval.
4)  A list of all staff associated with the study who will have access to the data you extracted.
5)  A signed MS Word Document   security agreement for all staff from Item 4.
6)  A completed, signed MS Word Document Data Warehouse Research Agreement.
7)  A data use list ( may be part of the study protocol - listing the data elements needed for the study.)
8)  A copy of the  MS Word Document Data Security Checklist for Principal Investigators.
9)  A copy of the  MS Word Document  Principal Investigators Certification: Storage and Security of VA Research Information.
10) A copy of the IRB submission detailing data use and requirements for protected health information (PHI).
11)  All projects extending beyond one year require that a copy of the IRB annual approval extension be forwarded to the Data Warehouse staff in order to retain access to the established account.

When all these items are in place, a Data Management user account will be created for the individual(s) named as the data extractor/analyst.
